Custom Field Trip Request Form

Give educators a clear booking request form that creates connected school, contact, and field-trip records in Salesforce and Veevart.

How it works

Ahead of field-trip bookings for the 2026–2027 school year, Kiewit Luminarium’s team needed changes that its out-of-the-box form could not support. I built a custom form on the museum’s WordPress website using HTML, CSS, and JavaScript. The form includes conditional logic, improved date and time controls, four guided sections, a final review step, accessibility-focused validation, and a design that matches the museum’s brand. Cloudflare Turnstile protects submissions. A Node.js Azure Function validates the data and securely creates the connected school, contact, and field-trip records in Salesforce and Veevart before the existing confirmation workflow takes over. Educators get one clear experience, while the Field Trips team receives the information in its existing systems.
